What help with overweight suits you? This questionnaire gives you personal advice in about five minutes. You will see your BMI, a healthy first goal in kilos and the routes that, according to the guidelines, fit your situation: from the free lifestyle programme via your GP (huisarts) to support alongside medication. Then you decide whether you want us to call you.

Transparency: for the programme, this site refers you to Overgewicht Nederland B.V., based in Amersfoort. We pass on the details from the form to them. The prices come from the provider itself (as of 21-10-2026). Everything about this site and the programme.

In brief

  • Four short parts: your goal, your profile, your health and then your advice.
  • Your answers stay on your own device. Only if you tick the box do we send them along with a call-back request.
  • The advice follows the NHG-Standaard Obesitas (2025), the Zorginstituut (2026) and the NVvH guideline (2020).
  • No medication is prescribed or sold via this site. A prescription always goes through your own GP (huisarts) or specialist.

How the weight loss test works

You answer one question per screen at a time. At the top you can see which part you are in and how far along you are. If you make a mistake, use the back button to go back a step. If you close the page by accident, your answers will still be there next time, on the same device. One button clears everything again.

The questions are about what you want to achieve, what you have already tried and what makes losing weight difficult for you. Then we ask for your height and weight so we can calculate your BMI. How to calculate it yourself and what the numbers mean is explained in Calculate your BMI and what your result tells you. Finally, a few questions about your health and any medication.

The Overgewicht Nederland questionnaire in four parts: your goal, your profile, health and your personal advice with sources
The four parts of the questionnaire. Advice according to NHG (2025), Zorginstituut (2026) and NVvH (2020).

You can skip any question you would rather not answer. That certainly applies to the question about your background. We only ask it because the guideline gives different thresholds for people with an Asian or Hindustani background, for waist circumference and for surgery.

What you get after the questionnaire

At the end, your personal result appears on one screen. You see your BMI and what it means. You also see a first goal: 5 to 10% of your weight. According to referral information on the GLI from Máxima MC, that amount of weight loss already brings considerable health benefits. We convert it into kilos for you, so you know what you are talking about.

Below that are the routes that match your answers, each with a short explanation, the source and a link to more information. You can print the result or save it as a PDF and take it to your GP (huisarts). If you would like our help, enter your name and phone number at the bottom. We will call you within one working day.

The routes the advice chooses from

The advice does not make up anything new. It compares your answers with the criteria that GPs and health insurers use. The table shows which routes there are and when they come into consideration.

If you are eligible for a GLI, that is almost always the first step. How to get there is explained in the reimbursed combined lifestyle intervention (GLI). If you already use medication, the advice shows what support alongside GLP-1 medication can do for you. If you want to know whether a temporary balloon is something for you, first read the explanation of how the gastric balloon works, its risks and costs. If surgery seems to come into consideration, we refer you to all the criteria for weight loss surgery (bariatric surgery), so you can prepare well for the conversation with your GP.

Why this check works differently

This questionnaire starts from the guideline and looks at which step suits you. Sometimes that is support from us, sometimes it is a conversation with your GP. And we say so.

  • No prices per medicine and no discounts. Advertising prescription medicines is prohibited. We name active substances by name, with brand names only in brackets so you recognise them.
  • No prediction in kilos. How much weight someone loses varies from person to person. We only give the first goal of 5 to 10% of your weight.
  • Safety first. If you use medication without a prescription, are pregnant or have severe abdominal pain, you will first get clear advice to contact your GP or pharmacy.
  • Sources with every piece of advice. You can always see where a criterion comes from.

Would you rather read about all the options first? The page with all obesity treatments puts them side by side.

What happens to your answers

The questionnaire runs in your own browser. As long as you do not send anything, your answers do not reach us. If you ask us to call you, we only send your name, phone number, email address and your choice of programme. Your answers about your health are only included if you tick a separate box for that. They are never passed on to an external form. More on this in the privacy policy of this site.

What support costs

The questionnaire and the advice are free. Per day: €3.55. If you then choose support, the price of Overgewicht Nederland B.V. applies: 12 months for €108 per month or €1,295 per year. There is also a 15-month research programme. Exactly what you get is described under personal support with overweight. Medication is never included. You discuss that with your GP.

Frequently asked questions about the check

How do I know which weight loss method suits me?

That depends on your BMI, your waist circumference, your health and what you have already tried. GPs (huisartsen) follow the NHG guideline on obesity (NHG-Standaard Obesitas) for this: lifestyle first, and only if that does not achieve enough, medication or surgery. The questionnaire compares your answers with those steps and shows which route makes sense now. It is advice, not a diagnosis.

How do I qualify for medical weight loss through the questionnaire?

The questionnaire itself does not give you access to medication or surgery, because these always go through your GP (huisarts) or specialist. It does show you which criteria apply to you. According to the NHG guideline (NHG-Standaard), medication becomes an option at a BMI of 35 or higher with a disease, or 40 or higher, after at least one year of combined lifestyle intervention (GLI) with less than 10% weight loss.

Is an online weight loss test reliable?

An online test is as reliable as the criteria behind it and the answers you give. This questionnaire uses the criteria of the NHG, the Zorginstituut and the NVvH, and states the source with every piece of advice. A doctor looks further than a questionnaire, for example at your blood values and your medical history. So take your result with you to your GP.

Why does the test ask about my waist circumference?

For the combined lifestyle intervention (GLI), it is not just your BMI that counts. With a BMI of 25 to 35, you are also eligible with a large waist circumference: 88 centimetres or more for women and 102 or more for men. For people with an Asian background, those limits are lower. If you do not know your waist circumference, skip the question. Your GP (huisarts) or practice nurse (POH) can measure it.

Why am I asked about my ethnic background?

Because the guidelines mention different limits for some groups. People with an Asian or Hindustani background are more likely to develop diabetes and cardiovascular disease at a lower weight. The NVvH therefore advises considering surgery for them from a BMI of 35. The question is voluntary. Your answer is only included if you tick the box yourself.

Which weight loss medication can a GP prescribe?

A GP (huisarts) can prescribe weight loss medication, but according to the NHG-Standaard only if a year of GLI achieved too little. Liraglutide (Saxenda) and naltrexone/bupropion (Mysimba) are reimbursed under certain conditions. Semaglutide (Wegovy) and tirzepatide (Mounjaro) are not reimbursed for obesity. More explanation is available at weight loss medication according to the GP guideline.

Can semaglutide as a diabetes medicine (Ozempic) be used to lose weight?

Semaglutide as a diabetes medicine (Ozempic) is registered in the Netherlands as a medicine for diabetes, not as a weight loss medicine, according to the inspectorate IGJ. For overweight, the relevant product is semaglutide (Wegovy), and that too is only available on prescription. Never order medicines without a prescription online. The IGJ explicitly warns against this.

What if I am already using weight loss injections?

Then the questionnaire asks which medicine you use, which phase you are in and whether you have side effects. You always decide your dose with the doctor who prescribes it. The advice focuses on what support can add: enough protein and exercise to preserve your muscles, dealing with side effects and preparing for the moment you stop.

How safe is weight loss with medication?

Medication and surgery have side effects and risks, and you weigh these up together with a doctor. With semaglutide, nausea, diarrhoea, constipation, abdominal pain and vomiting occur in 10 to 30 out of 100 people, according to Apotheek.nl. That is why the questionnaire also asks about side effects, pregnancy and other medication use, and refers you to your GP if in doubt.

How many kilos is a healthy first goal?

A healthy first goal is 5 to 10% of your weight. How much weight someone loses with a GLI varies from person to person; according to referral information on the GLI from Máxima MC, losing 5 to 10% of your weight already brings considerable health benefits. That is why the questionnaire result shows that goal in kilos. The GLI itself lasts 24 months and does not count towards your compulsory excess (eigen risico).

Is weight loss help covered by basic health insurance?

The combined lifestyle intervention (GLI) is: it is included in basic health insurance (basisverzekering) with no compulsory excess (eigen risico) and no personal contribution, according to the Zorginstituut. Liraglutide and naltrexone/bupropion are reimbursed under certain conditions. You pay for the Overgewicht Nederland B.V. support programme yourself. Check with your insurer whether your supplementary insurance (aanvullende verzekering) covers anything.

Can I fill in the questionnaire if I am under 18?

You can look at it, but the advice is intended for adults. The criteria for weight loss surgery apply from age 18, and the support from Overgewicht Nederland B.V. is for adults. If you are younger, talk to your parents and your GP or youth doctor (jeugdarts). They know the help that is available for young people.

Should I tell my GP that I took the test?

You don't have to, but it does help. Print your result or save it as a PDF and take it to your appointment. Your GP (huisarts) will then immediately see your BMI, your goals and what you have already tried. Tips for that conversation are in help with overweight via your GP.

Where can I go for help with overweight?

Your GP (huisarts) or practice nurse (POH) is the first point of contact and can refer you to a combined lifestyle intervention (GLI), a dietitian or a specialist. You can also choose support yourself, such as the Overgewicht Nederland B.V. programme in Amersfoort. Which combination suits you depends on your situation. That is exactly what the questionnaire result shows.

What happens after I send my details?

According to Overgewicht Nederland B.V., a staff member will call you within one working day. You discuss your situation and ask your questions. Only if you choose a programme will you receive the terms in writing, and you start after signing. Filling in the questionnaire or requesting a call-back comes with no strings attached.
